What is OpenEMR?

OpenEMR joins the clinical record and the business operation of a medical practice rather than treating them as separate products. Staff can schedule and check in patients, document encounters, manage medications and orders, communicate through a patient portal, and continue into coding, claims, payments, and operational reporting from the same installation.

OpenEMR is a free and open-source application for electronic health records and medical practice management. It combines patient records, scheduling, electronic billing, and internationalization in one system.

Core capabilities

  • Manage charts, encounters, forms, orders, prescriptions, and clinical documents
  • Coordinate scheduling, patient flow, portal messaging, and reminders
  • Handle coding, insurance, claims, payments, reports, and standards-based APIs

Clinical and administrative work share context

The application spans patient demographics, appointments, encounters, forms, medications, orders, documents, portal access, and reminders, then carries the same patient and encounter context into coding, insurance, claims, payments, and reports. That breadth suits practices seeking one deployable operational record.

Standards and configuration open the system

Role controls, configurable layouts, multilingual support, modules, reporting, and APIs let implementers shape the application around local requirements. The project documents FHIR, SMART, OAuth2, CCDA, and other interoperability work alongside its general API and deployment guidance.

Operated by the deploying organization

OpenEMR can be installed on infrastructure selected by the practice or delivered through service providers. The deploying organization therefore controls hosting, updates, backups, security configuration, integrations, and support arrangements rather than depending on a single mandatory hosted edition.

Where it fits

Use cases

  1. 01

Run a clinical visit from arrival to follow-up

Schedule and register the patient, document history and findings, record diagnoses and plans, create orders or prescriptions, and make appropriate information available through the patient portal after the encounter.

  2. 02

    Connect care delivery to billing

Carry encounter diagnoses, procedures, insurance details, fees, claims, remittances, and patient balances through one practice-management workflow instead of re-entering the same visit across unrelated systems.

  3. 03

Adapt an EHR to a local practice

Configure forms, layouts, languages, modules, roles, reports, and integrations for a specialty or regional workflow while retaining access to the source code and the project's community and professional support options.
